Patient Portal
Patient Login Instructions:
Patients now have 24/7 access to their confidential information from our secure patient portal. This portal access is secure and meets all compliance requirements outlined by the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
This portal will also allow you to update and view the following:
- Contact information
- Medical History
- Consent Forms
As of June 1, 2017 we have a NEW patient portal called MyPatientVisit. You will no longer be able to access our old portal.
The website link below will take you to the login page for MyPatientVisit.
To create an account, please click Entrance to Patient Portal. Select the option “Create an Account”, under “DON’T HAVE AN ACCOUNT?”. Please register with the same information (first name, last name, email address, etc) that you have provided to our office.
You will need a Security Code (individualized for each patient) and Practice ID, listed below. The Security Code will be emailed to you upon request. Once you are in the portal you will see your dashboard for MyPatientVisit.

Please rest assured that all information provided is kept confidential, secure, and HIPAA compliant. Please call the office at 919-286-7903 if you have any questions or haven’t received an email with your Security Code.
