Insurance:
If your insurance is with a participating plan (see list below) we will contact your company to verify the amount of your deductible and/or copayment. We cannot always obtain all your insurance information before or during your first visit. Therefore, you may be asked to pay for your first visit and we will file insurance for future visits. Overpayments will be refunded.
If you have insurance with which we do not participate, you will be given all the necessary claim information to file directly with your insurance company.
